Volunteer Application for the Liberal Arts in Prison Program

This form is due by noon on Tuesday, April 22nd, for all volunteers hoping to teach in the summer or fall of 2013. All volunteers must fill out this form.

Background check forms are also due by noon on Tuesday, April 22nd.

Orientations

You must attend the orientation at the institution where you wish to volunteer, even if you've already completed orientation at another institution. You cannot begin volunteering until after you've been through orientation.

I need to attend orientation at NCF, and will attend on Tuesday, April 30th or early in the fall semester. (Travel time 5pm-9:15pm)

I need to attend ICIW orientation, and will attend at a later TBD date (Travel time 3:30 pm - 10:00)

I need to attend orientation at the Iowa Juvenile Home. (time TBD)

I already attended NCF orientation.

Transportation

I am at least 20 years old and willing to drive a campus car. (You must have had a valid license, and no moving violations, for the past two years.)

I have a personal vehicle, or can borrow a friend’s car, and I would be willing to drive to prison. You will receive reimbursement for gas.

I don't have a car and I am ineligible to become a campus driver.

Shirt Size

We require all volunteers at NCF and ICIW (but not IJH) to wear red polo shirts. Please note your size, and guess larger if you’re guessing.
