Individual Student Files
Parents or guardians will be asked to sign a release of information form if the preschool requests that information be shared with another agency. Parents may withdraw permission at any time.
To meet the guidelines for the Department of Human Services each child at Grinnell College Preschool has a confidential file containing the following information:
- Application form
- Emergency care release form signed by the parent
- Permission form for field trips, photographs, videotaping and general research projects
- Physical health form signed by a physician
- Current information about health insurance required for emergency treatment
- Certificate of immunization card signed by a physician or public health official
- Current emergency contact information
- Names of persons authorized to pick up the child
- Instructions or health plans for special heath needs - allergies, asthma, seizures, sensory issues or other chronic conditions that may require medication or modifications or technology support
- Pick up and drop off permission form
- Accident reports when needed
- Other pertinent information
Throughout the year it is important that the preschool staff be informed of illnesses, injuries, allergies, changes in family situations and any other information that will help make the child's preschool experience a positive and beneficial one.
Solicitation
Parents will sign permission to share their name, phone number and address on a class list. The list of Grinnell College Preschool families may not be used for any form of solicitation.
Unlimited Access
In accordance with the Iowa Administrative Code 109.5, parents are "afforded unlimited access to their children and to the provider caring for their children during the center's hours of operation or whenever their children are in the care of the provider, unless parental contact has been prohibited by a court order."
Parents and legal guardians are welcome at preschool at any time to visit, observe, supervise on field trips, or demonstrate hobbies or crafts while preschool is in session.
