Service Transportation Request Form
Please fill out the following form if you would like to request transportation to and/or from a service project. 2 or more people must be traveling together. Transportation is not guaranteed and is arranged on a first come - first served basis. Requests must be made at least 2 business days in advance of the date transportation is needed. However, the earlier you request transportation, the more likely a vehicle and/or driver will be available. Once transportation has been arranged, an e-mail will be sent to you confirming your reservation. If no transportation is available you will also be notified. Changes in reservation requests are HIGHLY discouraged. Please plan ahead.
Important note:
Using a certified student driver is definitely encouraged. In order to be certified to drive campus vehicles, students must be at least 20 years old, have had a clean US drivers record for the past two years and complete a certification process through Facilities Management. Upon successful completion of the certification process students will be given a certified driver card by FM. The person reserving your vehicle will confirm that your requested student driver has this card to prior to moving forward with your transportation request.
