Chaplain's Office Transportation Request Form

Please complete the following form if you would like to request transportation to and/or from a religious or spiritual service,event, or activity. Once transportation has been arranged, an e-mail will be sent to you confirming your reservation. If no transportation is available you will also be notified.

Requirements:

  • 2 or more people must be traveling together.
  • Requests must be made at least 2 business days in advance of the date transportation is needed. However, the earlier you request transportation, the more likely a vehicle and/or driver will be available.
