New Student Health Requirements Fall of 2020

Updated deadline information for new student health forms:

First year students planning to live on campus for fall term 1:

  • Your deadline to complete all health requirements has been extended to August 10th.  
  • We ask that you submit your information as soon as it is completed in order to meet your requirements. 
  • Any students who have not submitted all health requirements by this date, will have a hold placed on their account.
  • Should a hold be placed on a student account, SHAW staff will reach out to you about steps you must take in order to get this hold lifted in time to register for classes through your student portal.  Keep an eye on the messaging feature.   
  • If for some reason you don’t hear back from us and you have a hold remaining on your account one week prior to registering for classes, please reach out to SHAW to get connected with one of our nursing staff.

  • We ask that you submit your information as soon as it is completed in order to meet your requirements.
  • Any students who have not submitted all health requirements by this date, will have a hold placed on their account.
  • Should a hold be placed on a student account, SHAW staff will reach out to you about steps you must take in order to get this hold lifted in time to register for classes through your student portal.  Keep an eye on the messaging feature.

Influenza vaccination requirements will be tracked separately. 

Make sure to get your flu vaccine during the upcoming fall season, either while on campus or back home, to ensure you are protected.  Don’t forget to upload your immunization record that includes this seasons influenza vaccine through the student portal.


Since the health and wellness of every Grinnell College student is of the utmost importance, the College has several health compliance requirements that all incoming first year students are required to meet.

These completed forms will need to be submitted to Student Health and Wellness (SHAW) by July 15, 2020. These requirements must be met before you are able to register for classes. View the Admission Requirement policy for 2020–21.

If you have questions about the vaccination requirements, please log in and send a secure message through the system (there will be an option on the left-side navigation called Messages) who will get back to you within the next 48–72 hrs. If you have a pressing situation and need to speak with someone directly, please call SHAW at 641-269-3230.

To Access the Required Health Requirements

  1. First, you must enroll in DUO.
    DUO is a security platform that provides a two-factor authentication, which adds a second layer of security for your online accounts. In order to access your student portal that houses your required health forms, you will need to enroll in DUO. Please follow the steps outlined in the message you received from the Office of Admission titled “How to Prepare for your Grinnell College Network Account” prior to step 2. If you need additional help enrolling in DUO, please contact the Grinnell College Technology Services Desk at 641-269-4901 or email ITS.
  2. Next, go to the Student Portal and enter your Grinnell College email username (without @grinnell.edu), then enter your password.
  3. Go to the Medical Clearances tab. The health requirements are submitted one of three ways: Below are step-by-step directions on the medical clearance section.
    • Some are forms within the Medical Clearance tab that you will complete and submit.
    • Others you will download, print, and complete with your healthcare provider and then upload into your student portal.
    • Others are just items that you will upload.

We recommend that you use Google Chrome internet-based browser. Please note that if you leave online forms idle for more than 10 minutes, you may be logged out.

    Items Required for Medical Clearance

    Health History — Please enter all known allergies, medications, medical conditions, family history, hospitalizations, and surgeries/procedures. If there is no applicable history for any section, select the green No button. Once each section has a response select the red Done button. All sections must have an entry in order to be compliant.

    Immunization Record — Upload the official immunization record provided to you by your medical provider or public health office.

    Measles – Enter the two dates that you received this immunization. Dates are located on your official immunization record. Each dose must be administered after 12 months of age to be considered valid.

    Med Health Eval — Log into the portal and visit this clearance section to download and print the Student Health Record form. Take this form to your provider for your required physical examination, make sure all sections of the form are completed, and the form is signed and dated by your health care provider. Once the form is complete, upload the form. All physical exams must be completed within one year of the first day of classes (August 28, 2019 – August 27, 2020)

    Meningococcal ACWY — Enter the date that you received this immunization. Dates are located on your official immunization record. The most recent dose must be within the last 5 years.

    Serogroup B Meningococcal — Enter the two dates that you received this immunization. Dates are located on your official immunization record. This immunization is typically given as a two-dose series with vaccines administered at 0 and 1 month (Bexsero) or 0 and 6 months (Trumenba) depending on the brand of vaccine. If you start the series with one brand your second dose must be with the same brand. Please start the series early so you may complete it prior to arriving in Grinnell or as soon as possible once at Grinnell to avoid registration holds in future semesters.

    Minor Consent — This form will be required of all students who are under the age of 18 as of August 1, 2020. Log into the portal and visit this clearance section to download and print the Minor Consent Form; have your parent/legal guardian review, sign and date the form; once complete upload form. If you do not see this item on your medical clearance list, it does not apply to you.

    Mumps — Enter the two dates that you received this immunization. Dates are located on your official immunization record. Each dose must be administered after 12 months of age to be considered valid.

    Rubella — Enter the two dates that you received this immunization. Dates are located on your official immunization record. Each dose must be administered after 12 months of age to be considered valid.

    SHAW Consent for Treatment — Read this form carefully and check the box next to “I acknowledge and agree to the following:” prior to clicking “Submit Final.” If you do not agree or have questions about the Consent to Treat form, please click “Cancel” and contact SHAW directly.

    TB Screening Form — Please answer all screening questions and click the Submit button. Based on your answers, further compliance items may appear on your Medical Clearances Menu. Be sure to check for additional requirements and complete as directed.

    Clinical Tuberculosis Assessment by Health Care ProviderIf directed on your Medical Clearances Menu, download and print this form, take this form to your provider for your required physical examination, then upload the completed form. Be sure the form is signed and dated by your health care provider.

    Tetanus-diphtheria — Enter the date that you received this immunization. Dates are located on your official immunization record. Most recent dose must be within the last 10 years.

    Varicella — Enter the two dates that you received this immunization. Dates are located on your official immunization record. Each dose must be administered after 12 months of age to be considered valid.

    Influenza — (2020-2021 season) - Enter the date of your 2020-2021 seasonal influenza vaccination. Flu vaccinations will be offered on campus free of charge to students while supplies last.

    Insurance Card upload — To add your insurance card, we encourage you to do this step through your mobile phone.

    1. From the menu on the left side of the Student Health Portal select Insurance Card.
    2. Select the Add New Card button. The View Insurance Card Window Appears.
    3. Type in the Member Name, Member ID, Group Number, Plan Number, and Plan Type.
    4. Take a picture on your phone of each side of the card.
    5. Select the Upload Front Image button, then select the image of the Front of the Card. Once the image is added you will be able to see the image that you selected. If the image appears correctly select the Looks Good button. If the image does not appear correctly select the Cancel Upload and select the correct picture from your phone.
    6. Once you have both images added select the red Save button.

    Health Insurance

    All students are required to have a U.S. based Affordable Care Act compliant
    health insurance policy while attending Grinnell College.

    • Make sure to bring your own insurance card with you when you come to campus, as this will be needed when utilizing services off campus.
    • If you don’t have your own card, request a duplicate card from your insurance provider.
    • If you participate in a health care plan such as Medicaid from a state other than Iowa, your plan will not transfer to Iowa. For questions regarding Medicaid in the state of Iowa and/or the student insurance plan, please contact Dustin Smith, our local insurance representative located at Ramsey Weeks, by calling 641-236-3141.
    • We are working on a way for you to upload your insurance information into the student portal. Look for this step to be added in the near future under the medical clearances section.

    Notification to Faculty/Parents/Legal Guardians

    If you fall ill and are going to miss class, it is important to email your faculty directly. We also encourage you to call SHAW and connect with one of your SHAW providers who can help guide your care moving forward. We offer a variety of services and are here to support you during your time of need. We understand that occasionally, your parents or legal guardians may want to speak directly with one of your SHAW providers regarding your health or mental health status. We are happy to speak with whomever you wish, as long as we have secured a written release and are able to discuss what you wish to share, doing so on a case-by-case basis. We do not honor blanket releases. While medical/counseling records within SHAW are kept confidential according to our policies, please note if you are hospitalized for a serious condition or injury, Student Affairs may reach out to your emergency contact to make them aware.

    SHAW Health Services

    Our student’s health and well-being is our main priority here at SHAW, and we enact this through a variety of services to help support students in their time of need. With the exception of modest, at-cost fees for a few medical tests/immunizations, these services are delivered free of charge. SHAW also offers free psychiatric services through telemedicine and in collaboration with the University of Iowa Hospitals and Clinics. Any medications prescribed, however, are obtained through the student's insurance plan.

    SHAW Mental Health Services

    Our goal at Student Health and Wellness is to provide access to counseling services, which help Grinnell students succeed academically. The services offered include educational outreach events, workshops, and group and individual counseling. We also offer support with case management and referrals off campus as appropriate. All mental health services offered to students on campus are offered at no charge to students.
